HYBRID HEALTH POLICIES
Health Documentation Requirements
Parents/guardians must provide:
- State-required immunization records
- Physical examination documentation when required
- Emergency contact information
- Medical action plans (e.g., allergies, asthma, seizures)
Illness Exclusion Policy
Students may not attend a hybrid site if they exhibit:
- Fever above 100.4°F
- Contagious illness
- Vomiting or diarrhea
- Rash of unknown origin
- Lice infestation
- COVID-19 symptoms per current guidance
Students may return once:
- Symptom-free for at least 24 hours
- Cleared by a physician when required
- Appropriate treatment has begun
Medication Administration
Medication may be administered only when:
- Written parent authorization is provided
- Physician orders are on file for prescriptions
- Medication is supplied in its original container
- Administration is logged appropriately
- Staff are trained and authorized
Health Emergencies at Hybrid Sites
Staff must:
- Assess the situation
- Notify school leadership
- Contact emergency services if necessary
- Notify parents/guardians
- Document the incident
- Follow established health or support plans
Nutrition, Physical Activity, and Wellness
Hybrid centers support student wellness through:
- Nut-free and allergy-aware food practices (as applicable)
- Clear food labeling and no-sharing policies
- Access to drinking water
- Movement breaks and physical activity
- SEL-aligned movement routines and recess when applicable
