Incident Reporting
All staff must report within 24 hours (immediately if urgent):
- Injuries
- Unsafe conditions
- Technology/security breaches
- Policy violations
- Bullying or harassment
- Threats or safety concerns
- Suspected abuse or neglect
Risk Assessment & Documentation
Annual risk assessments evaluate:
- Physical safety
- Cybersecurity vulnerabilities
- Legal compliance gaps
- Financial risks
- Insurance sufficiency
- Staff capacity risks
- Crisis preparedness
Documentation includes:
- Risk mitigation plans
- Incident logs
- Compliance records
- Safety drill documentation
